PLEASE LIST ANY DATES YOU WILL NOT BE AVAILABLE FOR REHEARSALS. The Academy Sessions/Rehearsals will be held on Tuesdays, Wednesdays, and Thursdays (9:30-2:00) on June 16,17, 18, 23, 24, 25, 26, 30, July 1, 2, 7, 8, 9. Times will be announced for rehearsals on July 13,14, 15, 16, 17. Leads/ensembles may have additional rehearsals during the week of June 1, week of June 8. For the success of the show, all cast members will be expected to attend the 12 Academy dates in June and July and tech week rehearsals (July 13-17). Performance Dates are July 18 &19. Participant Consent Participant Consent I will accept any role, participate in all scheduled rehearsals, and learn my parts, dances, and songs. I understand that if I miss a majority of rehearsals, it could mean I could be removed from my role. I will pay attention during rehearsal, be helpful to others, and be respectful to everyone during rehearsals and performances. I agree to these guidelines.

Parent Consent Parent Consent I will provide/arrange for transportation for rehearsals/performances and will be on time to drop off/pick up my child. I will inform the directors of any absences as far in advance as possible. I give permission to the Huber Opera House to post pictures online of my child during rehearsal for the purpose of publicity of the production. I will provide some clothing for costumes and agree to pay program fee ($150) *
